Youth FIRST Athletics — Parental Consent & Acknowledgment

Effective date: August 17, 2026

This is the form a parent or guardian signs (electronic signature) when enrolling an athlete in the Youth FIRST Athletics app. Each numbered item appears in the app as a checkbox; items 1–4 are required, item 5 is a required election (yes or no), and item 6 is informational.

1. COPPA consent (required)

☐ I am the parent or legal guardian of this athlete. I have read the Children's Privacy: COPPA Direct Notice to Parents and the Privacy Policy, and I consent to Youth FIRST Athletics, LLC collecting, using, and disclosing my child's personal information as those documents describe. I understand that:

• if my child is under 13, they have no login and all their information is managed through my account;
• if my child is 13–17, their own login works only after I approve the link from my account;
• I can review my child's information at any time, delete it, or refuse further collection, and my consent covers only what the notice describes — extra features (public reel, athletes-only chat, XP/rank, fueling pulse) each need their own opt-in.

2. Florida Parents' Bill of Rights acknowledgment (required)

☐ I acknowledge that Youth FIRST Athletics recognizes my rights under the Florida Parents' Bill of Rights (Fla. Stat. § 1014.04) to direct the upbringing, education, and health care of my minor child, including access to and control over my child's records held by Youth FIRST Athletics. Nothing in the app overrides those rights.

3. Liability waiver (required)

☐ I have read and electronically signed the Youth FIRST Athletics Liability Waiver presented with this enrollment. I understand my signature and the waiver version are stored, and that if the waiver changes materially I will be asked to sign the new version.

4. Medical and emergency information (required)

☐ I confirm the medical notes, allergy information, physician, and emergency-contact details I provided are accurate, and I will keep them updated. I understand the coach relies on them to keep my child safe.

5. Photo/video release (required election — choose one)

☐ YES — I grant the photo/video release described in the Photo/Video Release document. Private family-and-coach use is always in place; anything public (the squad reel) additionally requires my per-item opt-in and coach approval, and I can withdraw at any time.
☐ NO — I decline. My child's photos/videos stay private to my family and the coach only, and nothing appears publicly.

Only a guardian can change this election later.

6. Team chat — how permission works (informational)

The app's team chat has three channels. The athletes-only channel (available to 13–17 accounts) is off by default and requires my separate guardian permission, re-confirmed every single time it is enabled. A profanity filter runs, the coach moderates, and I can read the athletes' channel at any time. Nothing is enabled by this form — I will be asked when and if it applies. (Discord is a separate, 13+, parent-visible platform outside this app.)

Electronic signature: by typing my name and checking the boxes above, I sign this acknowledgment. My signature, the document version, and the date are stored in the consent audit log, which the coach can produce on request.
